Replacing a Lost or Stolen Jamaican Passport (Adult)

Your will be required to obtain and present a police report for the replacement of your lost or stolen Jamaican passport.  You will be required to submit the following documents:

Completed application form

The official police report. Please note that, on occasion the Consulate General of Jamaica , New York, may have to provide applicants with a sealed letter to present to the Police Station in order to obtain the Police Report, which our Office will facilitate.

Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate

Two (2) identical Photographs ( NB//: one photograph must be certified by the same certifying official who certified your application)

A valid Government issued Photo Identification (Example: Drivers Licence, Voter Identification).

Proof of name change if name has been changed (Marriage Certificate, Deed poll, Divorce Decree, Court Order)

The turnaround time will be approximately 6 months. At the same time, the Consulate General of Jamaica, New York, and the Passport, Immigration and Citizenship Agency, reserve the right to request additional and / or updated documents in order to ensure the completion of the process. 

 

Cost for replacement or lost or stolen Jamaican Passport (Adult):  USD $200.
Individuals in need of a letter from the Consulate General to obtain a police report must complete the necessary lost passport report and pay a fee of $30. This fee covers the administrative processing and issuance of the required letter.
The acceptable forms of payment are cash, or debit card (a 4% surcharge is applied by the banking machine used for debit card) for in-person appointments, Only.
For mailed in applications, payment must be made by way of Money Order made out to the Consulate General of Jamaica or the Jamaican Consulate General.

 

Replacing a lost or stolen Jamaican passport (minor (under 18yrs))

 

Your will be required to obtain and present a police report for the replacement of your lost or stolen Jamaican passport. You will be required to submit the following documents:

If the parent/Legal guardian is out of Jamaica then a written letter of consent giving the guardian permission to apply for and collect passport (notorized by an authorised person from the country they are in) is to be done and sent along with a copy of their passport or acceptable ID to the adult who will be doing the application through our office. 

 

Replacing a lost/stolen or damaged minor (under 18yrs) passport

Your will be required to obtain and present a police report for the replacement of your lost/stolen or damaged passport. You will be required to submit the following documents:

  • Completed application form
  • The official police report.  Please note that, on occasion the Consulate General of Jamaica , New York, may have to provide applicants with a sealed letter to present to the Police Station in order to obtain the Police Report, which our Office will facilitate.
  • Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate for the minor
